Thank you to everyone who joined us for the CommunityCares “On the Road Series” focused on Arizona’s Closed Loop Referral System with @uniteushq

This gathering highlighted new ways organizations can get involved in the referral network, strengthen cross-sector partnerships, and ensure community members are connected to the care and resources they need. If your organization is interested in learning more about these programs or how to get connected to the referral network, please contact chris@hotfood.org.

It was a wonderful opportunity to connect with community organizations, healthcare partners, and local leaders who are all working toward stronger collaboration and better support for individuals and families across Northern Arizona. Together, we explored how coordinated referrals, partnerships, and new community-based programs can help address social determinants of health and improve access to essential resources — including opportunities for providers to connect clients to programs that offer meal support and food access services. Thank you to all who attended!
